Can Health Insurance Really Be $0 a Month? What “Free Health Insurance” Means

HealthQuoted EditorialPublished Aug 26, 20263 min read

Some eligible Marketplace consumers may find an available plan with a monthly premium reduced to $0 after an advance premium tax credit. That is not guaranteed, and a $0 premium does not mean every healthcare service or prescription is free.

Educational information. Check current rules and the exact plan before making a coverage decision.

Why a plan may show a $0 premium

The Marketplace may allow an eligible household to apply some or all of its advance premium tax credit to the monthly plan premium. If the credit is at least as large as the premium for a particular available plan, the displayed amount owed for that premium may be $0.

Eligibility and amounts depend on current law, projected annual household income, tax household, location, ages, and other application information. Plan prices and availability vary, so no website or agent should guarantee free health insurance before the official determination.

$0 premium is not $0 healthcare

Most plans still require cost sharing when covered care is used. Read the Summary of Benefits and Coverage and plan documents before enrolling.

  • Deductible: the amount paid for certain covered services before the plan begins paying
  • Copayment: a fixed amount for a covered visit or item
  • Coinsurance: a percentage of an allowed covered cost
  • Out-of-pocket maximum: the annual limit for covered in-network cost sharing
  • Network and formulary rules: which providers and prescriptions receive the plan's covered rates

Compare affordable health insurance responsibly

A low monthly premium may be valuable, but it should be compared with likely use. Someone who expects regular specialist care or prescriptions may care more about copays, drug tiers, and the deductible than someone who mainly wants protection against unexpected major costs.

Check whether preferred doctors, hospitals, and prescriptions are covered by the exact plan. Carrier names alone do not confirm network or drug coverage.

Keep the Marketplace application current

Advance premium tax credits are reconciled on the federal tax return. Report material household and income changes to the Marketplace, and keep the annual Form 1095-A with tax records. HealthQuoted does not provide tax advice.

Common questions

Is $0-premium health insurance a scam?

Not necessarily. A legitimate Marketplace tax credit can reduce the premium for an available plan to $0 for some eligible applicants. Verify through HealthCare.gov or an approved enrollment channel and review the plan's other costs.

Will doctor visits also be free?

Not automatically. The plan may have deductibles, copays, coinsurance, network restrictions, and prescription costs.
